Employee Tuition Program Forms Due Jan. 13

TAMPA, Fla. (Dec. 3, 2010) -- To participate in USF's Employee Tuition Program, employees must complete the Employee Tuition Program Form and submit the completed and approved form to the Division of Human Resources in SVC 2172 by the fourth day of the term, January 13, 2011. If the form is not submitted or is submitted after the fourth day of the term, the employee’s registration in the class will be cancelled for non-payment. The form can be e-mailed to lfrazier@admin.usf.edu, faxed to 813-974-5227, sent via inter-office mail to SVC2172, or brought in person to the Human Resources Office.
